Article IV. COIN-OPERATED AMUSEMENT MACHINES |
§ 10-123. Application for license.
(a)
Application for license under this article shall be filed in writing on a form to be provided by the city and shall specify, at a minimum:
(1)
The name and address of the applicant, and if a firm, corporation, partnership or association, the principal officers thereof and their addresses;
(2)
The address of the premises where the licensed machine or machines are to be operated, together with the character of the business as carried on at such place;
(3)
A list of the amount and type of machines to be maintained on the premises including a copy of the state-issued license for each machine;
(4)
The name and address of the owner of the machine or machines, if other than the applicant, including a copy of the state master license assigned to that owner;
(5)
Whether or not the person having the management of or supervision over the business has been convicted of a crime of moral turpitude, has ever been found guilty of violating this article, has ever been found guilty of violating city or county ordinances or has ever been found guilty of violating the state law governing bona fide coin-operated amusement machines. If so, state the date of the violation, the date of conviction and the punishment assessed.
(b)
The applicant must list any other licenses or permits from the city held by the applicant including the license number.
(c)
The applicant must list any other businesses owned or operated by applicant within the city limits of the City of Riverdale.
